During your first visit, we make sure to obtain important background information, such as your dental and medical history, provide a comprehensive dental exam, and give you time to ask questions and get to know us.

If you have a complex dental history, have ongoing or unresolved dental issues, or have had dental xrays taken within the past 3 years, please download and complete the Authorization Form below and return it to us so we can obtain your previous dental records on your behalf – we will do the legwork for you!

Insurance and Financial Information

We accept any dental insurance that allows you to see any dentist you choose, typically called PPO plans. We are in-network with Delta Dental Premier, Cigna Total DPPO, United Concordia ELITE and Guardian Plus networks. We do not accept any HMO or DMO plans, which confine you to a specific dentist or list of dentists. Should you have any questions about your dental plan, please feel free to call us and we will be happy to verify your eligibility at our office.

We will estimate your dental insurance benefits and you will be responsible for the difference between the treatment fee and the insurance estimate at the time of service; however, you will remain responsible for the entire fee should your insurance plan pay less than estimated.

You can pay for your office visits and any treatments by cash, check, credit or debit card (Visa, Mastercard, Discover, and AMEX accepted), or we offer longer term financing with Care Credit.
